Child Support Form Criteria

In the fall of 2024, the Arizona Department of Child Safety (DCS) mailed letters to parents who once had an open dependency and/or severance case.

Please be aware that not all documents may pertain to your situation.  You are encouraged to Visit a Law Library for general guidance.

Prior to filing for a child support you will need the following:

  • Time and patience
  • Names and dates of birth of children
  • Gross income of both parents
  • Spousal maintenance paid or received by either parent
  • Court-ordered child support for other children (if applicable)
  • Medical, dental, and vision insurance costs for children
  • Educational expenses
  • If parenting time (custody) is not equal, the amount of days per year each parent is with the youth
  • Court-ordered arrears paid by the noncustodial parent

If submitting a Request for Title IV-D Child Support Services with the Department of Child Support Services, you will need the following:

  • Application is to be completed using BLACK ink
  • Valid identification
  • A copy of
    • Birth certificates for all children involved
    • Social Security cards
    • Marriage licenses (if applicable)
    • Pertinent death certificates
    • All court orders/modifications that are related to the child support request
    • All payments, related to your case, that have been made outside of ATLAS
  • Current or last known location information for the other person
